URGENT CARE
CARE COORDINATION
If you have chronic conditions or your care needs to be referred to a specialist, we will work as your medical liaison with your care team to chart the best course of care for you and your family.
CCH’s HIPAA compliant telemedicine software platform facilitates prompt and convenient face-to-face visits with your provider, taking the focus off of scheduling and placing it where it belongs – on your health, lifestyle management, or medication questions.
Direct Access
Whether you have a medical concern or question or need to schedule an in-person or telemedicine appointment, you can expect to see your provider within 12-24 hours of reaching out.
Preventative Care
Rather than waiting for a problem to arise before contacting a care provider, CCH believes in working to build a relationship with our patients to bridge the gap that so often exists between the medical and well community. This relationship is the key to tailoring a detailed healthcare plan for each of our patients based on their lifestyle and wellness goals.
Urgent Care
We are committed to addressing all of our patient’s urgent care needs as expeditiously as possible. Your provider will appropriately triage your adult and pediatric urgent care needs through either telemedicine or an in-home visit.
Telemedicine
Although, Clark Concierge Healthcare providers work with your PCP when appropriate to triage any referrals or care coordination, insurance prohibits us from replacing your primary care providers.
If your CCH provider is unavailable due to illness, vacation or other reason, communication with a covering provider will be arranged.
